Application forms
The following forms need to be completed to apply for inclusion in the regist
- Application Form Word form , completed by the applicant
- Further Information Sheet Word form, completed by the applicant
- No-conflict-of-interest Declaration Word form, completed by the organisation coordinating the external review of the applicant
The following documents have to be submitted:
- Terms of Reference of the external review
- Self-evaluation report provided for the external review

Optionally, the following documents can be submitted:
- Applicant's statement on the external review report (if any)
- For ENQA full members: Confirmation of ENQA membership, including correspondance between the Applicant and ENQA, if any
Please submit the application form by fax or regular mail, and all other documents only by email and in Portable Document Format (PDF) to application[at]eqar.eu.
Should you prefer to send your documents encrypted (with PGP or GnuPG) you can download a Public Key from the Contact
- In order to report substantive changes after your agency has been admitted to the Register, please refer to the information and template provided under "Reporting and renewal".
