Application forms
The following forms need to be submitted to apply for inclusion in the register:
- Application Form Word form , completed by the applicant quality assurance agency and submitted by fax or regular mail
- Further Information Sheet Word form, completed by the applicant quality assurance agency and submitted by email, fax or regular mail
- No-conflict-of-interest Declaration Word form, completed by the organisation coordinating the external review of the applicant and submitted by email, fax or regular mail
The following documents should be submitted by email:
- Terms of Reference of the external review
- Self-evaluation report provided for the external review

Optionally, the following documents should be submitted:
- Applicant's statement on the external review report (if any)
- For ENQA full members: Confirmation of ENQA membership, including correspondance between the Applicant and ENQA, if any
Please note: Should you choose to submit any documents other than the three forms by regular mail this can only be accepted if sent in 18 copies.
